Payment Policy

Payment is due in full at the time of visit. Dr. Klearman accepts cash, checks, Visa, MasterCard, or American Express.

Cancellation Policy

Patients who do not cancel their appointment within 24 hours will be charged a $50.00 cancellation fee.

Insurance Coverage

Dr. Klearman accepts insurance.  Prior to your visit, Dr. Klearman will verify your insurance benefits for you.  If your insurance does not cover her services, she will provide you with a 35% cash discount which is reflected below.

Pharmacy items prescribed by Dr. Klearman are not covered by insurance and will be charged to the patient.

Cost of Services

Prices listed are for those paid in full at the time of service, which is 35% off of fees billed to insurance.
Intitial Consultation $150
Additional Consultation and Treatment $100
Anti-aging Facelift $200/visit.  A minimum of 10 sessions is recommended.
Pharmacy items are additional.
