Payment Policy
Payment is due in full at the time of visit. Dr. Klearman accepts cash, checks, Visa, Mastercard, or American Express.

Cancellation Policy
Patients who do not cancel their appointment within 24 hours will be charged a $50.00 cancellation fee.

Insurance Coverage
Dr. Klearman is a provider for Cofinity and Blue Cross/Blue Sheild Insurace Companies.  However, not all plans cover for the services provided.  Please check with your insurance company prior to your visit to fully understand your benefits, and Dr. Klearman will bill your company appropriately. 

For out-of-network services, payment is due in full at the time of service.  If you desire, Dr. Klearman will provide you the necessary codes for you to submit your claims. You will receive a 35% discount off of your services when paid in full before leaving the office.

Pharmacy items prescribed by Dr. Klearman are not covered by insurance and will be charged to the patient.
